Doctor’s Notes
There are different type of notes available at an additional charge for patients. To obtain a note the patient must make an appointment beforehand with your family doctor, as they may need to document the illness in your medical chart. The family doctor may advise the employer about the impact of your illness or injury and suggest ways your to improve your work environment.

- Return To Work/Fitness Note
